Feedback
If you’ve been in touch with a Six Degrees Service, either as a patient yourself or as a carer or friend of someone else, please tell us how it was. We value your feedback and use it within our service to celebrate the things we are doing well and improve those things that we’re not doing so well at.
There are three ways to give us your feedback:
OPTION 01: Care Opinion
How does Care Opinion work?
Your story will be handled confidentially by the non-profit service Care Opinion. Your story may appear (without your name) on their website. Care Opinion asks the health service to respond to the feedback you share, but does not reveal your identity.
This service is independent of Six Degrees Social Enterprise and the NHS. For more information about Care Opinion and how your story is handled, visit the Care Opinion website
OPTION 02: Patient Experience Questionnaire (PEQ)
What is the PEQ?
All patients who have attended at least three sessions with us will be automatically sent a text message asking for their experience of their treatment. You can follow a link to the questionnaire which includes a few questions about your experience and free text boxes for you to include your own thoughts and comments about your time with us. You can also ask to speak to a member of staff to give your feedback verbally or if you need additional support, for example to use an interpreter. All feedback gathered through the PEQ is linked to your treatment record. If you do not wish to receive this text message, please let your practitioner know.
OPTION 03: Friends & Family Test
What is the Friends and Family Test?
The Friends and Family Test is a nationwide initiative to gain feedback from patients about the care and treatment they receive. As an NHS commissioned service, we use the Friends and Family Test to see whether you would recommend our service to your friends and family if they needed similar care or treatment. We also want to know where we have not met expectations so that we can make improvements. Your feedback is our best way of knowing where we are doing well or where we could do better.

The questionnaire asks one multiple choice question and two free-text questions. There are also some additional questions about you including your age and ethnicity, but these are optional.
How will the responses be used?
All questionnaire responses are anonymous; we evaluate the responses to see what we are doing well and where we can make improvements. Data gathered by Six Degrees is also sent to NHS England who publish scores their website every month.
Does the Friends and Family test replace Six Degrees’ complaints procedure or other forms of feedback?
No, this will not replace our current complaints procedure or other forms of feedback.

If you have a complaint then in the first instance we would encourage you to speak to the staff member you have spoken with. If this doesn’t resolve the problem then please ring us or email mailto:sixdegrees@nhs.net giving us the details of your complaint and someone will get in touch to discuss it further.
